Potentially Fatal Choking Hazard That First Aid May Not Dislodge: FDA Warns Against Once Upon a Farm Toddler Soft-Baked Bars, All Flavors and Lots, Firm Agrees to Recall

The U.S. Food and Drug Administration (FDA) on October 30 advised consumers not to buy or serve Once Upon a Farm Tractor Wheels Organic Toddler Soft-Baked Bars, citing a potentially fatal choking hazard. The advisory covers all flavors, quantities and varieties of the product, including all lot codes and expiration dates.
The FDA said it has received several reports describing toddlers aged 1.5 to 2.5 years suffering complete airway obstruction and being unable to breathe after consuming the product. When chewed, the product reportedly forms a dry, sticky mass that standard choking interventions, including back blows and the Heimlich maneuver, may fail to dislodge. Some reported choking incidents required emergency response intervention and hospital observation.
The product is sold nationwide online and at stores including Costco, Target and Walmart, and may also be sold through Amazon and other third-party vendors. The FDA said it will update the advisory as more information becomes available.
The FDA has informed the firm of the adverse event reports, and the firm has agreed to initiate a recall. Consumers should not buy or serve any flavor of the bars and should immediately throw away any they have; retailers should not sell the product.
The FDA also advises calling 9-1-1 if someone is choking and other first aid measures have failed to dislodge the obstruction. Consumers with symptoms should contact their health care provider, and complaints or adverse events can be reported through the FDA's Industry and Consumer Assistance channel. The FDA said it will continue to update the advisory as information becomes available.
