The Cigna Group Q3 Revenue Rises 6% to $73.7 Billion, Raises 2026 Adjusted EPS Outlook to at Least $30.60

The Cigna Group (NYSE: CI) reported third quarter 2026 results on November 5: total revenues rose 6% to $73.7 billion ($73.68 billion), while shareholders' net income was $1.4 billion, or $5.47 per share, compared with $1.9 billion, or $6.98 per share, a year earlier, primarily reflecting the absence of an after-tax gain on sale recognized in the third quarter of 2025.
Adjusted income from operations was $2.1 billion, or $7.98 per share, compared with $7.83 per share a year earlier. "Our strong third quarter results demonstrate how we are building on this momentum by making health care more affordable and personalized, while advancing Accelerate to One to drive productivity, reduce costs, and support sustained long-term earnings growth," said Brian Evanko, President and Chief Executive Officer of The Cigna Group.
By segment, Evernorth Health Services posted adjusted revenues of $63.77 billion, up 6%, with adjusted income from operations before tax of $1.83 billion, down 4%: Pharmacy Benefit Services income fell 22%, reflecting large client contract renewals and strategic investments, while Specialty and Care Services income rose 15%. Cigna Healthcare's adjusted revenues rose 9% to $11.71 billion and its adjusted income from operations before tax rose 15% to $1.19 billion, with a medical care ratio of 85.0%, versus 84.8% a year earlier.
Total medical customers reached 18.45 million at September 30, up 2% year over year, including 16.70 million U.S. healthcare customers, while total customer relationships were 177.6 million. Through November 4, the company had repurchased 1.8 million shares for approximately $500 million, and its debt-to-capitalization ratio stood at 41.8% at September 30.
Cigna raised its full-year 2026 outlook for adjusted income from operations to at least $30.60 per share, up $0.15 from its prior projection. At its 2026 Investor Day, the company introduced its "Lead to One" vision and long-term financial targets, including 10%-14% adjusted EPS compound annual growth and roughly $50 billion in cumulative operating cash flow through 2030, alongside the multi-year, $3 billion "Accelerate to One" modernization and productivity initiative.
