San Francisco Issues Fatal Overdose Alert for Kratom-Derived Opioid MGM-15: First Local Death Recorded in September

The San Francisco Department of Public Health issued a health alert on October 21 warning providers and the public about MGM-15 (dihydro-7-hydroxymitragynine), a semi-synthetic kratom-derived opioid not approved for any medical use that can cause fatal opioid overdose.
The Office of the Chief Medical Examiner detected MGM-15 in an adult who died in late September, the first known San Francisco case; the substance had previously been identified in overdoses in Utah and Washington. The DEA has designated MGM-15 a Schedule 1 substance, making its sale illegal. It has been found in products marketed as 'cat's claw' (uncaria tomentosa) sold at smoke shops and gas stations; forensic testing confirmed MGM-15 in the 'Script botanicals x Maui labs' product the decedent used, whose packaging made no reference to kratom or 7-hydroxymitragynine. The medical examiner's office will continue testing for novel substances including MGM-15 and share results with the health department.
