Man Charged Over $12.89 Million in Medicaid Payments Allegedly Diverted From Phoenix Children's Hospital

State prosecutors in Phoenix, Maricopa County, Arizona, have filed a complaint in Maricopa County Superior Court alleging that $12.89 million in Medicaid payments meant for Phoenix Children's Hospital was diverted to a bank account controlled by Juan Carlos da Cunha Tcachman, 56, who faces two Class 2 felony charges, fraudulent schemes and artifices and theft. He had not been arrested as of October 20, according to the Arizona Attorney General's Office.
The complaint says the alleged scheme began in July 2026, when AHCCCS, Arizona's Medicaid program, received a faxed letter purporting to be from Wells Fargo claiming the hospital had changed its bank account effective July 1. After a staff member spoke by phone with someone identified as “Michelle,” AHCCCS approved a change to the electronic transfer information, and three payments of about $4.29 million each, totaling $12,891,725.10, were sent to the account. Some of the money has been recovered.
Investigators say Tcachman was the account's holder and signer, opened it using a District of Columbia passport, and made multiple large withdrawals in August and September. The Attorney General's Office said the case and investigation are ongoing and it could not comment further. The charges are allegations and have not been proven in court.
