FDA Warns: Once Upon A Farm Tractor Wheels Toddler Bars Pose a Potentially Fatal Choking Hazard

The Food and Drug Administration warned consumers on October 30 not to buy or serve Once Upon A Farm Tractor Wheels Organic Toddler Soft-Baked Bars, describing the snack as a potentially fatal choking hazard for young children.
The FDA said it has received several reports of toddlers between 1.5 and 2.5 years old suffering complete airway obstruction and being unable to breathe after eating the bars. When chewed, the product reportedly forms a dry, sticky mass that standard choking interventions, including back blows and the Heimlich maneuver, may fail to dislodge. Some reported incidents required emergency response intervention and hospital observation. The FDA informed the company of the adverse event reports, and the firm agreed to initiate a recall; the company said its decision followed three consumer complaints and that it plans to reformulate the product.
The recall covers every flavor, quantity and variety of the Tractor Wheels bars, including all lot codes and expiration dates: Apple, Sweet Potato & Spinach; Strawberry, Pumpkin & Beet; and Banana, Pumpkin & Cauliflower. The bars have been sold nationwide at stores including Costco, Target and Walmart and through online retailers.
The FDA advises consumers not to buy or serve any of the bars and to throw away any packages at home immediately. No other Once Upon A Farm products are affected, and the agency says it will keep updating its advisory as more information becomes available.
